The following documents are important to help the ASC better understand your complaint:

  • Correspondence to and from the company or individual(s)
  • Account statements
  • Account applications
  • Advertising, promotional, or sales materials
  • Notes/emails on conversations with the company or individual(s)
  • Forms you signed
  • Cancelled cheques
  • Offering documents
  • Subscription Form

Upload is limited to 10 MB per file and a maximum of 10 files.
File formats accepted are .png, .jpeg, .jpg, .pdf, .pdfx, .doc, .docx, .xls, .xlsx, .html, .txt.

It is not recommended to upload documents using your mobile device.
